Program Eligibility
Member Requirements
- Must be on Medicaid or be Medicaid Eligible
- Require assistance with Activities of Daily Living (ADL)
- Must need 5 or more hours of care
- Be a participant of CCSP or SOURCE Waiver Program
Caregiver Requirements
- Must be 18+ years old
- Be a family member or friend (excluding spouses)
- Must be committed to taking care of a member
- Must be biologically related to the member
- Must live with the person as a primary caregiver
