Are you or someone you know over the age of 65, on Medicaid, and enrolled in a Commonwealth Care Alliance / CCA SCO plan? You may be eligible to receive home care services in Massachusetts.
Program Eligibility
Member Requirements
- Must be 65 years of age or older
- Require assistance with at least 1 Activity of Daily Living (ADL)
- Must have Medicaid (or be eligible)
- Must be enrolled in a qualifying SCO health plan
Caregiver Requirements
- Must be 18+ years old
- Be a family member or friend (excluding spouses)
- Complete required training and orientation
- Pass background checks (CORI, SORI, OIG)
- TB screening within the past 12 months
