Program Eligibility
Member Requirements
- Must be 18+ years old
- Require assistance with at least 1 Activity of Daily Living (ADL)
- Have Michigan Medicaid (or be eligible for Medicaid)
- Complete an assessment with an Adult Services worker to determine approved services and hours
Caregiver Requirements
- Must be 18+ years old
- Be a family member or friend (excluding spouses)
- Must be registered in the CHAMPS system
- Pass background checks (CORI, SORI, OIG)
